Scheduling an Appointment
To schedule an appointment with Su, you can email or call our office, or visit the Contact Us page and complete the form at the top of the page. The quickest way to reach us is by email.
New Patient Visits
If you are a new patient, please visit our Forms page and complete the forms under "New Patient Forms." Fill out either the adult history form or child history form depending on whether you are coming in or you are bringing your child in for a visit. The first visit typically exceeds the time of a normal appointment.
We only accept cash or check payment.
We do not accept insurance. Only certain insurance plans cover appointments and diagnostic tests we offer. We do not want to be limited in how we treat our patients, and we want to make sure that the patient is the priority, not the insurance coverage. It is for these reasons that we do not accept insurance.
All payments must be made by cash or check. We will give you a copy of the statement for the visit which includes ICD-10 code(s) through which your insurance may reimburse you.

Rescheduling an Appointment
Please give us 48 hours notice before canceling/rescheduling an appointment. If cancellation is made less than 48 hours prior to appointment, a cancellation fee of $100.00 will be charged to the patient. Missing an appointment will result in payment of the visit in full.

We offer patient packages with predetermined appointment schedules and pricing. The Sattva Family team takes the commitment to you and your family seriously. Over the years we have seen that patients and families with regular and structured care typically have the best outcome and experience.
